护理程序的四个步骤

　　Student： They are ： assessment of patient； planning of care； implementation of care and evaluation of care given.

　　T： What do you do when you assess the patient？

　　S： We observe the patient using the senses such as looking ， listening， touching， smelling an speaking. An active assessment of the patient's needs.

　　T： What is Nursing diagnosis？

　　S： Nurasing diagnosis is a statement of an actual or potential patient problem that requires nursing intervention.

　　T： How do you make the plan of care？

　　S： The care plan is built on the foundation of patient's problems. The highst priority will be designed as problem number one and treated first. For example the oxgen ned of a patient with myocardial infarction must always be met at once.

　　T： What is the meaning of implementation？

　　S： That is to carry out the plan.

　　老师：请讲一下护理程序的四个步骤。

　　学生：这四步是：对病人的估计；制定护理计划；实施护理计划和对护理工作的评价。

　　老师：你在估计阶段都做什么？

　　学生：运用感觉器官观察病人，如：望、听、触、嗅和语言，要对病人的需要做一个切实的估计。

　　老师：什么是护理诊断？

　　学生：护理诊断是确定病人现存或潜在的需要采用护理措施的问题。

　　老师：怎样制订护理计划？

　　学生：我们以病人的问题为根据制订护理计划。确定急需解决的问题，要首先予以解决。例如心肌梗塞病人的氧气需要总是必须立即满足的。

　　老师：实施计划是什么？

　　学生：意思是将计划付诸实践。
